Case Manager - Community Health (SDOH)

Hours: 8:00 - 5:00 Monday - Friday, 40 hours per week 

Hiring Range: $26.00 - $28.00/hour 

The Case Manager performs professional care management services independently, providing education, referral and support to pregnant women with identified high risk factors. The Case Manager will collaborate with Prepaid Health Plans, Pregnancy Medical Homes/OB providers and other community partners to ensure program goals are being met. 

Primary Responsibilities: 

  • Assess member needs, develop person centered care plan and goals related to the risk factors identified with the member 
  • Provide member education , referral and follow-up based on need, risk or condition to ensure that goals are achieved
  • Coordinate service plans with other agencies as required by member need 
  • Document in Virtual Health electronic records system on a daily basis 
  • Conduct Nurse Newborn Postpartum home visits per program requirements
  • Complete person centered visits via phone, in home, at school and in other community settings 
  • Carry out referrals for service to CHA, ancillary agencies and community resources 

Education/Experience: Graduation from an accredited four year college or university with a B.S. degree in nursing or graduation from an accredited school of professional nursing with two years of professional nursing  experience. Public Health with experience in case management required. Maternity, care management, home visiting experience preferred. 

License: Valid North Carolina driver's license. Must have active license to practice as a Registered Nurse in   North Carolina. 


  • Ability to be extremely flexible work independently, work as part of a team and possess good decision making skills 
  • Possess ability to plan and work collaboratively with community partners 
  • Thorough knowledge of and skill in the application of theory, practices, principles and techniques employed in the field of public health and related programs
  • Considerable knowledge of and skill in obtaining a health history and performing patient assessments sufficient to determine if there is any deviation from normal
  • Strong knowledge base in maternal and postpartum issues
  • Considerable knowledge of current social and economic problems relating to healthcare
  • Strong knowledge of available community resources and organizations
