(ALLIANCE POSITION)

  

TITLE:                     COMMUNITY SYSTEMS CHANGE CONSULTANT

                        (Innovative Approaches Coordinator)                  

 

DUTIES:  This is a consultative position to improve the community-wide systems of care to best meet the needs of families of children and youth with special health care needs (CYSHCN).  The consultant staffs and maintains a steering committee of community agencies and parents to assess system issues affecting families of CYSHCN, facilitates the development of an action plan driven by parent and provider data, facilitates the completion of action items and assists with all evaluation efforts.

 

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:  Bachelor’s degree in sociology, psychology, political science, business administration, public administration or related field, from an appropriately accredited institution and four years of experience related to the specific program assignment; or an equivalent combination of education and experience.

 

SALARY: Expected annual salary range based on Community Development Specialist II, Salary Grade GN 13, $48,000 - $52,000.
