TITLE:            INNOVATIVE APPROACHES PARENT OUTREACH COORDINATOR

                                               

DUTIES:  This position performs outreach activities to engage parents May 8th, 2021

of CYSHCN, and recruit their active involvement in the Innovative Approaches Initiative.   Position works collaboratively with parents, primary care providers and community agencies to improve the system of care for children and youth with special health care needs up to age twenty-one.  Carries out Action Plan for IA which addresses education and support needs for parents and caregivers of CYSHCN through the project; information and support for care providers and community agencies serving CYSHCN regarding available resources and how to access/navigate the service system.

 

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:  Graduation from a four-year college or university with at least 1 year of experience in community programs; or an equivalent combination of training and experience.  A minimum of 4 years of experience working with families of children and youth with special health care needs is preferred.

 

SALARY: Expected annual salary range for the part-time IA Parent Outreach Coordinator using salary grade GN09 as a guide, would result in an hourly salary range of $17.63 - $27.05 ($18,338.50 - $28,135.50).  Compensation will be based on level of experience and competency.
