NC Minority Diabetes Prevention Program (NCMDPP) Grant

 

Service Area:  Cherokee, Clay, Graham, Haywood, Jackson, Swain, Transylvania Counties and the Eastern Ban of Cherokee Indians (EBCI).

The purpose of the NCMDPP initiative is to create healthier communities by:

  1. Engaging at risk populations for participation in Diabetes Prevention Programs (DPP)  
  2. Screening at risk populatinos for pre-diabetes
  3. Referring to Diabetes Prevention Programs
  4. Offering the Diabetes Prevention Program to minority communities in Region 1

Description of work:

• Provide technical assistance related to local NCMDPPs throughout the region
• Ensure that the NCMDPPs achieves quality performance outcomes
• Ensure participant and program data is entered into the NCMDPP database on a weekly basis and submit monthly reports to the NC Office of Minority Health and Disparities (NC OMHHD) Program Contact
• Participate in all mandatory NC OMHHD coach calls
• Conduct a targeted marketing and awareness campaign
• Coordinate screenings for prediabetes
• Refer at risk people to local DPP classes
• Organize and co-host community forums to raise awareness of prediabetes
• Provide DPP classes with items and services to address participant barriers to participating in the DPP classes; including transportation, childcare, supplies, etc.
• Administer an incentive program for participants
• Complete required evaluation and program activities

 

The primary purpose of this position is to create a Regional Collaborative which will engage, screen and deliver the NCMDPP that includes the CDC curricula:  Prevent T2" or "Prevenga el                                                                                                                                    T2" to minority communities within the region.

Background and Education:

  • Master's degree in public health or health and/or human services field and one year experience in health education/health promotion or a                                                                                                                                                                                                         bachelor's degree and two years experience in a community, business, or governmental program, preferably in the fields of health                                                                                                                                                                                                education/health promotion; or an equivalent combination of  training and experience
