POSITION:          Public Health Nurse II

RECRUITMENT STANDARDS:

The Pregnancy Care Manager (OBCM) is a health professional who is responsible for coordinating specialized multi-disciplinary services to an assigned caseload of high risk Medicaid pregnant patients by utilizing nursing assessment skills and knowledge of prenatal, postpartum/newborn care and community resources. The major goals of this program are to reduce infant mortality through improved pregnancy outcomes; ensure the provision of comprehensive preventive health services to pregnant women and their infants; and assist pregnant women to meet other priority needs that affect their well-being and that of their families.  The Pregnancy Care Manager (OBCM) receives referrals from Health Department Providers as well as other community OB providers. Once the referral is received, the client is assessed for existing or potential risks and needs and a care plan is developed and implemented based on these needs. This position serves primarily Medicaid patients receiving care through either public or private provider. OBCM will follow up on referrals in a timely manner and keep provider abreast of changes in the patient's status. All contacts with patients, providers and referral agencies will be thoroughly documented in the electronic medical record (Virtual Health).

 

WORK EXPERIENCE REQUIREMENTS:

Considerable knowledge of and skill in the application of nursing theory, practices, principles, and techniques employed in the field of public health and related programs; considerable knowledge of available resources and organizations and the ability to coordinate these as needed; general knowledge of current social and economic problems relating to public health.

 

EDUCATION REQUIREMENTS:

Graduation from an accredited school of professional nursing and two years of professional nursing experience including one year in public health; or an equivalent combination of education and experience.  Current license to practice as a Registered Nurse in North Carolina by the North Carolina Board of Nursing. Must possess a valid North Carolina driver’s license.

SALARY:  Grade 32
