NCAPHA is actively recruiting for a Public Health Nurse II, Case Management placed at Transylvania County Health Department.

Full-time community nurse position functioning as a Care Management for High-Risk Pregnancies (CMHRP) and Care Management for Health Risk Children (CMARC). This requires weekly, monthly, or quarterly contacts that could be made by phone, face-to-face contact in the home or other settings.

This position is responsible under Medicaid Managed Care for assessing the needs of high-risk pregnant women and at-risk children (birth – 5 years). The position will be responsible for developing a service coordination plan of care for the individual and family. The plan of care requires knowledge of community services and provides follow-up that ensures goals have been achieved. To meet the needs of the client, visits may occur at regularly scheduled medical appointments, in community settings, or in the home. This may occasionally require work outside normal business hours. The position requires documentation of client-agency contacts and meeting the caseload benchmarks as issued by Medicaid Managed Care contracts. All documentation is done using the electronic case management database. The position requires knowledge of obstetric conditions, pediatric conditions, perinatal conditions, and excellent communication/teaching skills.

Knowledge’s, Skills and Abilities:

Must be able to understand program requirements and applicable laws to provide direct care and education based on this knowledge. Accuracy is required in performing assessments and recording these observations and the care rendered in the medical record. Must be able to provide instructions to clients orally and in writing. Must be able to communicate program guidelines to other agencies and the community.

Required Minimum Training:

Graduation from a four-year university or college with a B.S. Degree in Nursing that includes a Public Health rotation and one year of Public Health Nursing experience; or graduation from an accredited school of professional nursing and two years of professional nursing experience including one year in public health or an equivalent combination of training and experience.

Individuals employed in a local Health Department without a B.S. Degree which includes a Public Health Nursing rotation or a Master’s in Public Health, will be required to complete the “Introduction to Principles and Practices of Public Health Nursing” course within one year of employment as a condition of continued employment in accordance with Health Services Commission Rule. Experience in a field of nursing that is closely related to the anticipated work assignment can be substituted for Public Health experience for hiring purposes; however, the employee will also be required to complete the “Introduction to Principles and Practices of Public Health Nursing” course or objectively demonstrate an adequate understanding of Public Health practice within 1 year of employment.


License or Certification Required by Statute or Regulation:

Licensed to practice as a registered nurse by the NC Board of Nursing

Current NC Driver’s License

CPR Certification
