PURPOSE OF SOUTHEASTERN NORTH CAROLINA HEALTHY START PROGRAM:

To provide outreach, case management/carecoordination, preconception, prenatal, and interconception health education,and support to women and infants who reside in Hoke County. These services will integrate evidence­ based screening tools, case management/care coordination methodologies, group educational sessions, and a home-visiting curriculum. Emphasis will beon providing services to African American women of reproductive age and their families.

PRIMARY PURPOSE OF POSITION:

  • To improve women's health during the preconception, prenatal, and interconception periods by providing case management/care coordination, home visiting, health promotion, education, and support services to women, children, and families up to 18 months after birth.
  • To improve birth outcomes and the health of infants up to18 months after birth.

The CHW will work under the direction of the Care Coordination Supervisor. (The Health Director will serve as supervisor while the Care Coordination Supervisor position is vacant.)

This position provides preconception case management/care coordination series to at least 20 unduplicated women and conducts at least four quarterly health education/support group sessions.

Assist the Maternal Health Coordinator with missed appointments, follow-ups, and access to health services.

 

WORK SCHEDULE:

Must be flexible to meet the goals of the SENCHSP grant.

Standard work hours are 8:00AM to 5PM (Monday through Friday); however, these hours are subject to change to meet the expectations of the grant. Other hours required to work may include early morning, evening, and weekend time. Hours worked beyond 40 hours must have supervisor pre-approval and are compensated with overtime pay.

ROLE AND RESPONSIBILITIES

  • Conduct outreach, exhibiting at health promotion events to share preconception health, pregnancy, and interconception health messages and information with women and men, and conducting targeted small group outreach presentations annually as specified in the agency contract/agreement.
  • Conduct bi-monthly health education/support group sessions for women of child bearing age and community members using the SENCHSP curriculum.
  • Assist the Care Coordination Supervisor in the completion of program participants' action steps, which are outlined in their care coordination plans. This can include assistance with access to health and human services, arranging and facilitating services (transportation, childcare, and translation services if required) to promote appointment compliance, and completing follow-up visits (as needed) to reinforce parenting skills, development, and behavior change using the SENCHSP curriculum.
  • The SENCHSP Community Health Worker will:
    • Follow up with prospective preconception program participants (within seven days of referral to the SENCHSP) to answer program-related questions, initiate enrollment, and obtain the participants' signatures on SENCHSP Letters of Agreement to acknowledge enrollment and participation in the program services.
    • Enrollment will include completion of appropriate Healthy Start Screening Tools in the ChallengerSoft Case Management Database and development of individual service plan. (The ChallengerSoft Case Management Database is a web-based case management system created to support Federal Healthy Start projects.
    • Ensure preconception program participants have a medical home through consistent case management.
  • Conduct monthly care coordination contacts for preconception program participants by phone or in person (in the office, clinic, or community location).
  • Conduct at least four quarterly health education/support group sessions per year for preconception program participants using the SENCHSP curriculum to provide education regarding the various contraceptive methods and other positive preconception reproductive health behaviors.
  • Documentation of education and reproductive life plan will be made in the ChallengerSoft Case Management Database.
  • Enter all required documentation into the ChallengerSoft Case Management Database by the 5th day of each month.
  • Participate in Care Management for High-Risk Pregnancy and Outreach Team meetings to provide feedback to the Maternal and Child Health (MCH) staff on the barriers clients face in utilizing MCH services and recommendations for improving services to meet clients' needs better.
  • Serve on the Hoke County Public Health Advisory Committee to help assess local health department policies and procedures for tracking and following upon missed maternity, family planning, and well-child appointments.
  • Document individual and group contacts in a daily and monthly activity log. Complete timesheets, monthly travel, and evaluation forms when needed and as required by the grant.

QUALIFICATIONS AND EDUCATION REQUIREMENTS

Working knowledge of applicable supportive mechanical acts and sanitation and personal hygiene; basic knowledge of the social, economic, and environmental conditions of clients to be served; ability to establish rapport and relate to the population served and a variety of service disciplines (nurses, doctors, social workers, health educators); ability to follow written and verbal instructions; ability to record essential data; ability to perform required physical tasks.

  • Demonstrated possession of knowledge, skills, and abilities as stated above through at least one yearof experience in performing similar tasks in a hospital, clinic, nursing home, or other medical facility.
  • Personal experience with pregnancy and parenting.
  • Graduation from high school or GED


LICENSE OR CERTIFICATION REQUIRED BY STATUTE OR REGULATION:

  • Valid North Carolina Driver's License with a good driving record
  • Proof of Rubella Immunity
  • Must have a reliable personal vehicle for use at work (Will be reimbursed mileage at the current county rate)
