POSITION SUMMARY:

 

The Cumberland County Department of Social Services (DSS) and the Cumberland County Department of Public Health (CCDPH) have received funding to implement an initiative which focuses on primary prevention by utilizing a whole person/family approach and addressing social determinants of health (SDOH). The initiative includes multi-disciplinary Case Management and Care Coordination teams responsive to community referrals to provide care coordination and case management services to address social determinants of health, address social needs, improve health behaviors and health outcomes, improve health equity, and reduce health disparities, reduce preventable hospital stays, and prevent and reduce child abuse and neglect.

 

The primary purpose of this position is to carry out federal and state laws governing the provision of foster care and adoption services to children and their families. (Public Law 96-272, Federal Adoption and Safe Families of 1997, the North Carolina Adoption and Safe Families Act, and North Carolina General Statute 131D-10). As a part of operationalizing federal and state mandates this position will provide services that promote stability, positive parenting, and permanency to children living in out-of-home placements.  Placements for these children may be on a temporary or long-term basis.   Worker in this position is responsible for participating in the recruitment, training, licensing, supervision, and retention of foster families and/or intake of child protective services reports.  They are also responsible for completing pre-placement assessments for foster families wishing to adopt, and for developing, maintaining and implementing a plan to ensure a permanent outcome for children being adopted by the foster family.  In addition, the worker in this position will be a part of a team that will develop and implement a plan for a child and family consistent with the best interest of the child, and the child’s safety.  Such plans may involve reunification with biological parents, relative placement, adoption, or independent living.

 

DUTIES AND RESPONSIBILITIES:

 

Duties for this position include, but are not limited to:

  • Assist with Informational Meetings conducted by the Recruiter for potential applicants to foster or adopt.  
  • May at times be responsible for conducting the meeting if the recruiter is unavailable.  
  • Assist with pre-service training for families wishing to become foster and adoptive parents.
  • May also assist with or directly provide 10 hours of training to foster parents wishing to be relicensed.
  • Will be responsible for assuring that foster parents help a child move from the foster home to other placement resources with the least amount of trauma.
  • Will be responsible for accepting and processing referrals of child abuse, neglect or dependency.  
  • Provide information on community resources to meet the needs of children and families.
  • Social workers in this position assist social worker I/AT’s by conducting diligent searches to locate families when a referral has been received.

MINIMUM QUALIFICATIONS:

  • Bachelor's degree in social work from an accredited school of social work;  
  • Bachelor's degree in a human services field from an accredited college or university and one year directly related experience; 
  • Bachelor's degree from an accredited college or university and two years directly related experience.

 

Directly Related Experience is defined as Human Service experience in the areas of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy and treatment planning.
